This Preliminary assessment form is to be filled in by each patient and is a necessity in order our Vaidya’s understand your medical conditions and can better assess your general state of physical and mental wellbeing. Based on your Preliminary assessment form inputs, you will be further advised by our Vaidyas on whether you are in a condition to undergo the intense curative course of treatments that are offered at our hospital.
