PRELIMINARY ASSESSMENT

This Preliminary Assessment Form is to be filled in by each patient and is a necessity for our Vaidyas (Ayurveda doctors) to understand your medical conditions and better assess your general state of physical & mental wellbeing. Based on your inputs, you will be further advised by our Vaidyas on whether you can undergo the intensive curative treatments on offer at our hospitals.
